 Why Your Health Center Should Apply? 

 As your health center works toward expanding your primary care services to include care for seniors living with HIV, you will engage in an established curriculum designed to align your existing work and responsibilities with changing and expanding demands. Further, the curriculum puts expansion of services in the context of the Patient Centered Medical Home (PCMH).

With PCMH as the guiding model and HIV as the population of focus, components of the curriculum include:

  • Maximizing revenue for HIV services/building the business case
  • HIV clinical care within primary care
  • Care coordination
  • Partnership development
  • Practice and system redesign/patient flow
  • Workforce development

The curriculum features highly-regarded, expert faculty and will be delivered to you in a variety of ways, including:

  • A dynamic online community that fosters shared learning and innovation through forums, real-time access to experts and data, and communication about relevant information and events

  • Face-to-face workshops
