Patient Centered Medical Home

The patient-centered medical home (PCMH) is four primary things:

(1)   the fundamental tenets of primary care: first contact access, comprehensiveness, integration/coordination, and relationships involving sustained partnership

(2)   new ways of organizing practice

(3)   development of practices’ internal capabilities

(4)   related health care system and reimbursement changes.


All of these are focused on improving the health of whole people, families, communities and populations, and on increasing the value of healthcare.


The value of the fundamental tenets of primary care is well established. This value includes higher health care quality, better whole-person and population health, lower cost and reduced inequalities compared to healthcare systems not based on primary care.

Program Assistance Letter 2011-01

This Program Assistance Letter (PAL) describes the Health Resources and Services Administration’s (HRSA) Patient-Centered Medical/Health Home (PCMHH) Initiative, which supports and encourages health centers to gain recognition under the medical home program offered in partnership with the National Committee for Quality Assurance (NCQA).  This PAL outlines the processes and requirements for applying for recognition and highlights the technical support available under the HRSA PCMHH Initiative.



Program Assistance Letter 2009-12

This PAL describes the accrediting organizations that are contracted to provide surveys and educational services under the Accreditation Initiative Program for the new three-year contract period (October 2009 – September 2012). Furthermore, this PAL outlines the processes and requirements for applying for accreditation and highlights the broad array of technical supports that are available under the Accreditation Initiative.
